Position Summary:
Reporting to the Chinook Primary Care Network (CPCN) Co-Chairs, the Medical Director is a key leadership position.

The Medical Director, in a dyad relationship with the CPCN’s Executive Director, is responsible for providing clinical leadership in the development of the CPCN, in a shared decision-making model between clinical and business leadership. This position provides a key link between the CPCN physicians, Alberta Health Services, Physician organizations and other stakeholders. This position also provides continuity of physician leadership over time, as elected physician members of the CPCN’s Board of Directors rotate through governance positions.

The role ranges from a 0.2FTE to a 0.4 FTE. Some months have higher demands, and the time commitment may flex up to 0.4FTE to fulfill those responsibilities, where other months require a lower time commitment.

Key Responsibilities:

Physician Resource:
  • Supports the continuing medical education sponsored by the CPCN;
  • Provides peer mentorship support to participating physicians as required;
  • Is available to CPCN physicians as an ongoing resource and support.
Physician Leadership:
  • Participates in the establishment, review, and revisions of policies, procedures and guidelines relating to clinic operations and patient care;
  • Plays a leadership role in advancing the interests of the CPCN with internal and external stakeholders, as directed by the CPCN Co-Chairs;
  • Works closely with the PCN’s Central Support Team in supporting best practices and innovative models of care in service planning, development and delivery;
  • Supports the coordination and use of clinical research in meeting the needs of the CPCN’s patient population;
  • Is a regular member of the CPCN’s Executive Committee and the CPCN Board of Directors;
  • Supports the CPCN Clinics to identify areas to improve clinical outcomes.
Dyad Partnership:
  • Works in a dyad relationship with the CPCN’s Executive Director to together provide clinical and business leadership in a shared decision-making model;
  • Provides input into the CPCN’s Business Plan.

Professional Qualifications:
  • The ideal candidate is an experienced physician with a proven track record as an innovative and creative leader, administrator and team player in the healthcare industry where they have led change. In addition, the candidate will:
  • Be licensed to practice medicine in Alberta;
  • Possess at least five (5) years of experience in family physician practice;
  • Maintain a minimum of 0.2 FTE time in a full spectrum of family physician practice within the CPCN (one FTE = 40 hours per week);

Job Description
The Clinical Care Coordinator (CCC) role combines quality improvement, program planning, and team development activities within a clinic environment to support the participation of the clinic in the Chinook Primary Care Network. 

Prairie Treatment Clinic (PTC) offers opioid agonist therapy (OAT), including methadone, suboxone and sublocade for individuals with opioid use disorder (OUD).

The Clinical Care Coordinator working in PTC will provide a patient-centered approach to ensure patients receive safe, supportive and confidential care while being treated for their OUD. PTC may be the first point of contact for some individuals who do not presently have a primary care provider. As patients receive treatment and support for their OUD, primary care may become more essential for their ongoing recovery. It may then be prudent to deliver some primary care and facilitate connection to other primary care providers.

While practicing within their scope of nursing practice, the CCC will work with other team members to facilitate quality improvement and best practice initiatives to provide excellent care to patients and indirectly to families and communities.  

Position Qualifications:  
  • Some form of post-secondary degree preferred and current licensure with any level of provincial Nursing College, in good standing is required.
  • Minimum three years’ experience in a primary care, community health, ambulatory care setting and / or mental health. Preference will be given to candidates with experience working to full scope of practice. 
  • Experience working with addictions / addiction medicine an asset
  • Knowledge or willingness to develop knowledge and skills in opioid use disorder and treatment

Job Description:
The Chinook Primary Care Network (CPCN) is seeking a dedicated Pelvic Health Physiotherapist to provide evidence-based practice that is in alignment with professional standards of practice, Alberta legislation and regulations, code of ethics and AHS policy and guidelines.

The Pelvic Health Physiotherapist will:
  • Provide rehabilitation for pelvic health issues such as urinary or fecal incontinence, diastasis recti, dyspareunia, post-partum return to activity, pelvic organ prolapse, post-prostatectomy incontinence, and pelvic pain.
  • Review clinical referrals, conduct comprehensive assessments, develop, plan and implement appropriate physiotherapy services.
  • Work both independently, and as part of an interdisciplinary team while maintaining effective communication with patients and colleagues.  
  • Participate in client, family and caregiver education; produce and maintain client information, documentation and verbal reports to support the delivery of physiotherapy services.
  • Maintain a practice knowledge base including knowledge specific to the defined service area, relevant services and resources and regulatory college standards and requirements.
  • Meet with Chinook PCN Clinical & Quality Improvement Lead on an ongoing basis to discuss learning, develop processes and define issues to be brought forward to ensure optimal outcome of this project.

Accountability:
The CPCN Pelvic Health Physiotherapist will work from the Lethbridge Pelvic Health Clinic located in the Breast & Cervical Health Clinic at the Chinook Regional Hospital and will be accountable to and report to the Chinook PCN Clinical & Quality Improvement Lead.

  • Completion of master's degree in Physiotherapy from an accredited university program or bachelor's degree in Physiotherapy from an accredited university program and related experience.

Summary of Work:
The Laboratory/X-ray Technician will perform laboratory tests and radiology examinations using appropriate requisition forms as ordered from authorized professionals in timely fashion. Technical duties and responsibilities: 
• Follow radiation protection practices, regulations, and philosophy to reduce risk to patients, staff and visitors.
• Knowledge of all applicable legislation and regulations involved.
• Perform phlebotomy procedures (macro and/or micro) in a manner both safe to phlebotomist and patient
• Provide patient instructions on various sample collections utilizing current supplied patient instruction sheets
• Book all patient referrals for ultrasound, CT, MRI, UGI, etc., to appropriate facilities
• Work closely with Clinic physicians, Clinical Care Coordinator, LPNs, Health Information Manager and other staff members to ensure steady patient flow
• Perform routine urinalysis and urine HCG testing according to methodology with results sent back to physician to assist with patient diagnosis
• Make every effort patient wait time for laboratory testing and radiology examinations are kept to a minimum
• Be accountable for ensuring that laboratory tests and radiology examinations are done with utmost accuracy
• Be able to recognize any errors regarding testing and contacting appropriate individuals
• Report all abnormal results and findings to appropriate individuals

  • Graduate from an approved Combined Laboratory & X-ray Program
  • CPR & First Aid
